You must carefully and completely enter all information into the required fields in the registration form before it will allow you to submit.  If you do not complete a required field, you will have to begin the registration process again.  Please take your time and take note of required fields.  In addition, please make every effort to gather necessary information to complete this form (donor addresses, name of your tree, etc.)

Sign Information:

This information is for the sign placed by your Tree. Accuracy is essential! Signs will be printed prior to Decorating Day; therefore, it is important you confirm participants and the spelling of their names. You may list up to three decorators and donors. Organizations or group names are encouraged instead of individuals.

Gift Certificates:

If you are including gift certificates with your tree, please provide temporary certificates for tree display only. The actual certificates will be collected on Decorating Day at Check In. Bring a printed list of gift certificates to be displayed with your tree on Opening Night. The list may not exceed 4”x6” and will be taken down after Opening Night.
